The Hospital for Sick Children Foundation
Use of this App is limited to specific individuals having a certain relationship with the Foundation. Each user authorized to use the App is assigned a userid and password by the Foundation. The app is not usable by the general public.
The following analytical data is collected by the App as it is used:

Password related events such as password change requests and attempts (both failed and successful)

Login attempts, including failures
The following are two types of data that a user may optionally provide in the App. The information (including timestamps) is uploaded to a Foundation server and used solely for the purpose of facilitating the Foundation’s donation activities.

Whether the user knows a particular individual identified in the App (yes/no logical response);

If so, whether such user would be willing to help connect a Foundation fundraiser with such individual (a free form text field is provided to capture any information a user may wish to provide)
The above information is linked to the user’s identity.
No other information is collected. There is no information that a user of the App is required to provide.
